Dr. Johansson's office accepts most major insurance plans, with the exception of Worker's Comp, Medicaid and their related plans.  Please call our office if you would like to confirm that we accept your health insurance plan.

For individuals who do not currently have health insurance, we can provide you with a fee schedule in advance. 
 

 
PLEASE NOTE THAT IT IS THE SOLE RESPONSIBILITY OF THE PATIENT, NOT THE DOCTOR, TO KNOW ALL DETAILS OF THEIR INSURANCE PLAN (INCLUDING CO-PAYMENTS, DEDUCTIBLES, CO-INSURANCE, AND NON-COVERED ITEMS).  THIS OFFICE DOES ITS BEST TO DETERMINE WHAT SERVICES ARE COVERED OR NOT, BUT WE DO NOT GUARANTEE THAT YOUR INSURANCE COMPANY MAY NOT HOLD YOU RESPONSIBLE FOR FURTHER CHARGES.  THESE AMOUNTS WILL BE DETERMINED SOLELY BY YOUR INSURER AND NOT BY THIS OFFICE, EVEN IF THE INVOICE COMES FROM DR. JOHANSSON'S OFFICE.  ***ALL QUESTIONS REGARDING THESE CHARGES MUST BE DIRECTED TO YOUR INSURER.***
 
Some insurance plans require that you meet a deductible before they will remit any payment directly to us.  You are responsible for paying this deductible.  If you do have a deductible and it has not been met, you will be required to pay $150 at your first visit and $50 for follow-up visits until your deductible is met.  If the insurance calculates that you owe us additional fees after this initial payment, we will send you an invoice for that amount. 
 
Many insurance plans also require that patients will have to pay co-insurance - a percentage of the allowed charges. 
 
Co-payments are payable at the time of your appointment, and may be paid only by cash or check.  We are not accepting credit card payments at this time, unless the charge amount is $100 or above.  If you have a flex-spending or HSA debit card, we will provide you with a detailed receipt for your payment so that you may be reimbursed this amount from your account.
 
* Please note that some insurance plans require a referral from your Primary Care Physician (PCP).  It is the patient's responsibility to make sure that the referral, if required, is received by our office before your appointment time.  If you are seen without the appropriate referral, you will be responsible for any charges incurred.  Please contact us if you have questions regarding your referral or how to obtain one.  We are happy to help.
